In many interviews and commentaries, I've read and heard that it was written as a trilogy.
You can see it on the Matrix Revisited DVD...when they are interviewing the head of WB (I believe he was the head), he said they approached him with the script, and saying it's a trilogy, but the head was like, "Lets take it one film at a time."
In the TIME article...Joel Silver recalls, "In the first version of the script, you actually saw Zion. But they didn't have the time or the money to do that. If the first film hadn't been successful, nobody would see the rest of the story. But the boys had it in their heads. So when the studio said, 'Let's make a sequel,' they had already planned a lot of it."
Then in another article, Ann-Moss said, "After the original was finished and WB was set to release a sequel, The Wachowski Brothers gave me the script, and it hasn't changed once. Not once."
So, during filming, they didn't say, "Hey, remember those screens in the first film? Lets connect it somehow in Reloaded!" "Yeah! Great idea Andy!" "I know Larry!"
All in all...I believe they had the entire story in mind from the beginning. Not every single detail, but I believe they knew the major plot points before the original.
If you don't believe it...oh well. Don't have to.

es gibt die Monitore des Architekten im 1. Teil.. ok daß ist nicht unbedingt ein Beweis dafür.. könnte ja auch einfach nur so im ersten Teil eingestreut worden sein..
und es gibt das Telefonat am Schluss, daß nachdem man Reloaded gesehen hat nur zwischen Neo und dem Architekten stattgefunden haben kann..
