Continuity: In the final scene of the film, as Sam returns home and his children rush out to meet him, the cows in the background change position each time the camera goes back to Sam.
Continuity: In the scene where King Theoden is dying under his horse after Eowyn has slain the Witch King, there are three spots of mud on Theoden's right cheek that are so clear that they almost appear to be a tattoo. The camera cuts to Eowyn, then back to Theoden, and the spots on his face are faint, smudged, and in different locations. The camera cuts again from Theoden to Eowyn and back, and the spots are back to their original locations, pristine and unsmudged.
Continuity: When the fellowship congregate around Frodo while he is recovering in bed. Aragorn and Legolas' line of sight is not correct, they are looking in the right direction but not at Frodo, Merry and Pippin.
Other: When The Steward of Gondor is about to burn Faramir on the pyre, Gandalf dramatically bursts through the doors on Shadowfax. Look at the rider's face - it is obviously not Gandalf, but a double.
Continuity: In the scene in which Gollum frames Sam for eating all the lembas bread, when Frodo is telling Sam to leave, the tracks made by the tears on Sam's face change from shot to shot.
Continuity: In the scene where Sam is returning the ring to Frodo in the orc tower, the number of fingers he has wrapped around the chain changes with each shot as the dialogue between the two characters progresses.
Continuity: In the opening scene with Smeagol and Deagol, they are standing on the bank of the river fighting over the ring. The ring falls from Deaol's muddy hand into the grass, and as the two collapse on the ground, both reaching for the ring, the ring is completely clean, despite being covered in mud in Deagol's hand only seconds earlier.
Continuity: The scene where Pippin and Gandalf are talking about "the end" in Minas Tirith, during the battle of Pelenor fields. In one close-up shot, Gandalf's sword blade is shiny and silver. In the next shot, it's coated in black orc blood, then in the next shot, it's silver again.
Continuity: In the scene where Frodo is tied up in the Tower, part of his face and hair is partly covered in spider webbing - the only opening is his face where Sam parted it to see that he had "died". After a few scenes of orcs, the next shot shows that Frodo has clean hair/face and his hands are still tied up.
Plot hole: How did Shelob's sting get through Frodo if he was wearing his mithril vest? Its obvious he wasn't stung higher or lower than where the vest was.
Continuity: At the end, after Elrond, Bilbo, Celeborn and Galadriel boarded the ship for Grey Havens, they are nowhere to be seen when the ship is behind Gandalf as he speaks to the hobbits and even when Gandalf and Frodo board the ship.
Continuity: In the battle where Aragorn's army is before the gates of Mordor, the dragons and eagles have a battle of their own in the air. In no subsequent shots is there evidence of the dragons that were killed and would have fallen to the ground below.
Revealing: In the first scene in Edoras (the capital city of Rohan). The first pan over Edoras: You can see the pan is shown in reverse, with the smoke going into the chimney and the fire at the end is burning backwards. The flags are fluttering oddly as well.
Continuity: Merry and Pippin drank Entish water (Two Towers Extended Edition) so they should be taller than Frodo and Sam, but when the 4 of them stand side by side in Minas Tirith they are the same height.
Continuity: When Pippin brushes past Gandalf the White to kneel down before Denethor and offer his own life, "such as it is," as a service to the King to repay the debt to Boromir in honor, the Wizard turns to his right to look at the Hobbit while calling out his name in an attempt to stop him, but his glance doesn't quite follow Pippin's move, which is understandable since the two actors don't actually share presence on screen at the same time (or at least distance from the camera).
